IMPORTANT - PHCS COMMITMENT FORM
All school districts in New York have been required to come up with plans for reopening schools in September following the closure in the spring caused by COVID-19.
For the purposes of planning, it is important that we know how you plan to have your students receive their education. Our plan calls for a hybrid model, with the whole student body divided into two cohorts (groups), and each of the cohorts attending school, in-person, two days a week, continuing their work at home the other three days. Parents who are not comfortable sending their students to school at this time may opt for full time remote learning for their students. Those students will receive lessons, materials, and support from district teachers.
If you choose to have your student(s) start the year with full remote learning, you may change them to the in-person hybrid model as time goes on. We ask that you provide the school at least two weeks notice prior to returning to the in-person model. Parents should contact their child's building level principal if they are interested in making a change to the model of instruction originally requested for their child.
Your student can only follow one of the two models, not both. So in the case of situations involving joint custody, we ask that you ensure that all parents are on the same page before you submit the form for the student.
**PLEASE COMPLETE A SEPARATE FORM LINKED BELOW FOR EACH STUDENT YOU HAVE ATTENDING PHCS.**
