Dear Parents and Guardians of PHCS Students 12-17 Years-Old,

St. Lawrence Public Health is offering first and second dose Pfizer vaccination clinics for children 12 years-old and up before school lets out for the summer. Here are two options for children to become vaccinated.

1)    Parents can take their children to the first dose vaccination clinic at the Leithead Field House on the St. Lawrence University Campus in Canton on Saturday, May 22nd from 9 AM – 4 PM (appointments are available for adolescents 12-17 years old AND for their accompanying parents/guardians). A second dose appointment will be scheduled at that time.

2)    PHCS will transport students from school to the AA Kingston Middle School in Potsdam on Tuesday, May 25th from 10:45-11:45 for their first Pfizer dose. The follow-up 2nd dose clinic will be at the same time and same location on Tuesday, June 15th and transportation from school will also be provided. 

Students 12-17 years old will need to have the consent form filled out by a parent or legal guardian.  The registration deadline is Monday, May 24th at 2 PM (do not worry if the times available on the online registration do not perfectly match the PHCS time slot, simply plug in the closest available appointment time).

These vaccination clinics are voluntary and free of charge. PHCS is transporting students as a convenience to families interested in vaccinating their children. Please find the parent consent form attached.
